To be cleared for CHS Sports, all Online Clearance Forms, Physical/Medical History, Athletic Fees and all prior years Fees/Fines/Obligations
must be submitted and paid/cleared before an athlete will be cleared to practice!

Along with completing the online Clearance Forms, athletes will also need to turn in the WIAA Medical History and Physical forms to the CHS Athletic Office. Click on WIAA Physical Form below to print the Medical History/Physical form.
